At some stage, it becomes difficult to elderly people to manage everyday affairs, especially if it is complicated by disabilities. To address the issue and help elderly people live their lives fully, the New York State Department of Health administers the Nursing Home Transition and Diversion Medicaid Waiver Program (NHTD), a Home and Community-Based Services program.

The NHTD program is meant to assist elderly people with disabilities to manage their everyday life. It helps to stay in the community, get all benefits of independent living and eliminate risks to their health. All services provided are funded by Medicaid.
